The Care Connection

The Care Connection is a program of care for people living at home and having aggressive curative treatment of a life-threatening illness.

Who Benefits:

  • A patient who is undergoing aggressive treatment of a life-threatening illness, with a life expectancy of 1 year or less.
  • A patient whose attending physician approves participation in the program.
  • A patient who lives in Garfield County or in a 60 mile radius of Enid .

Care is provided regardless of age, sex, race, sexual orientation, national origin, color, creed, disability or disease.


Services Provided:

Social Worker- The social worker will make an assessment visit followed by regular visits as needed.  The goal of the social worker for The Care Connection patient includes becoming their advocate in making community resources available.

Nurse- An assigned nurse will make regular visits every 2 weeks.  A nurse will be available 24 hours a day, 7 days a week for emergencies.

Volunteer- A trained volunteer will provide support and friendship for their patient and his/her family.  The volunteer is available on a regular or as-needed basis.

Chaplain- The Chaplain will visit as requested by the patient to provide spiritual care and support.

When a person is faced with a life-threatening illness there are many needs and problems that arise.  It is the goal of The Care Connection to help people connect with resources to meet those needs.

The Care Connection is a service of Hospice Circle of Love, provided without charge to the patient/family.  No third party payer will be billed.
