Social
worker
The
social worker will make an assessment visit followed by regular visits
as needed. The social worker
becomes the patient’s advocate in making community resources
available.
Nurses
An
assigned nurse will make regular visits as needed.
A nurse will be available 24 hours a day, 7 days a week for
crisis and caregiver support. Nurses
are always available to answer patient/caregiver questions.
Eight of our nurses are nationally certified in hospice and palliative
care.
Nurse’s
Aide
Aides
provide respite for the family and personal care for the patient.
Five of our nurse aides are nationally certified in hospice and
palliative care.
Chaplain
The
Chaplain will visit as requested by the patient to provide spiritual
care and support. They also
provide grief counseling for the family.
Physical
Therapist/Occupational Therapist
Physical
and occupational therapists assist patients to be as mobile and
self-sufficient as possible. They
can teach exercises, safety in transfers and more.
Speech
Therapist
A
speech therapist assists with exercises intended to improve speech and
make swallowing easier.
Music
Therapist
Therapists
use music to address physical, psychological, cognitive and/or social
functioning. This non-invasive medium is directed towards reducing
pain and anxiety and assisting with other end of life issues.
Dietician
When
needed, a dietician assists the family in addressing the patient’s
nutritional needs.
Counselor
A
counselor is available on an as-needed basis to assist in helping the
family and patient to process their grief issues.
Volunteer
A
trained volunteer will provide support and friendship for the patient
and his/her family. The
volunteer is available on a regular or as-needed basis.
Medical
Director
Dr. Brian Whitson, Dr. Christopher Shearer and Dr. Jami Benton attend
interdisciplinary team meetings, take patients that transfer from out of
the area and take care of any prescription needs or problems when a
patient's primary doctor is unavailable. Dr. Whitson is the only
physician in Northwest Oklahoma to be board certified in hospice and
palliative care. |
